Custom Design Forms
Last updated:
A Custom Design Form is a contact form built into Sign Customiser. Use it to collect custom design requests that the app cannot generate automatically.
The form appears in the customiser:
-
On desktop, it opens from the Custom tab. This is the default tab name. You can rename it.
-
On mobile, it appears as a link below the selections.

Customers can upload files, write messages, and use dropdowns, checkboxes and multiple-choice fields. They do not leave the page while they fill in the form. After they submit, they stay on the page unless you choose Redirect to a page (see Step 4).
To quote customiser designs, use Quote Mode instead. For more information, read Quote Mode.
Before you start
The form shows on the Custom tab only when all of these are true:
-
Embed a form is selected on the Custom Design page (see Step 5).
-
At least one form has the Custom designs type. A Quote Mode form does not create the tab.
-
Your plan includes Custom Design Forms. If Embed a form is not available, your plan does not include them. Upgrade your plan to use the feature.
The text editor setting (Features → Editor Modes) controls the Text and Custom switch. If it is off, customers go straight to the form and see no switch.
Note: If you turn on the AI Sign Designer, it replaces the Embed a form choice. The Custom tab then opens the AI Sign Designer, not your form. When the AI cannot make a design and Send designs that cannot be made automatically to the fallback form is on (the default), it sends the customer to the fallback form that you choose. The fallback form needs a File Upload field and a required Email field. For more information, read the AI Sign Designer Guide.
Create and edit a form
This video shows how to create and edit a form.
Step 1: Add a new form
-
Go to the Sign Customiser admin:
-
Universal subscribers: Go to Home in your account.
-
Shopify users: Open the app from your Shopify admin.
-
-
Click Customisers in the menu.
-
Select the customiser you want to edit.
-
In the left-hand menu, go to Features → Forms.
-
Click Add Form. If you have no forms, click Add New Form.

-
Under Form Quote Type, select one of these:
-
Custom designs (Unique sign designs beyond letter signs) – For design requests that your customiser cannot handle.
-
Quote Mode (Quoting designs based on customiser options) – For quotes on designs that customers build in the customiser.
-
-
Enter a Label for your form. Customers see it only when you have more than one form.
-
(Optional) Add a Description and change the Submit Button Label.

Step 2: Add fields to your form
-
In the Fields section, click Add new field and select the field type you need. For a detailed walk-through, follow Add and Edit Custom Form Field Options.

-
Every form must contain a required Email field. New forms include this field. Do not remove it or make it optional.
-
(Optional) For Shopify users only: Turn on +Shopify Sync in the Fields section. Submitting the form then creates a customer in Shopify. The Email field already syncs. To sync names, click Add new field and add First name and Last name fields. On a new form, First name is added when you turn on +Shopify Sync, so add only Last name.
Step 3: Set a receiver email (optional)
By default, form submissions go to the shop owner’s email address on Shopify, or to the Store owner’s account email on Universal.
-
To send them to a different address, enter a Receiver Email in the Email section of the form. If you leave it blank, Sign Customiser uses the default email for your store (see above).
-
Change the Email Subject if you want. If you leave it blank, the subject is “Custom Design”. Sign Customiser always adds the submission number and the customiser ID before the subject.

Step 4: Choose what happens after submission
In the After submission section, choose one of these actions:
-
Show thank you message (default) – A thank you message appears.
-
Redirect to a page – Customers go to a URL you enter in Redirect URL.

To change the thank you message, go to Features → Forms. Edit Form Success Message in the Form Settings card. The default text is “Thank you for your submission.” This message applies to all forms in the customiser.
Click Save to apply your changes.
Step 5: Show your form in the customiser
-
In the left-hand menu, go to Core Setup → Custom Designs.
-
In the Custom designs card, select Embed a form. If AI Sign Designer (recommended) is selected, the form does not show. Selecting Embed a form replaces it.

-
Click Save.
The form now shows in the customiser if every condition in Before you start is true.
Step 6 (optional): Listen for form submissions with JavaScript
Your own JavaScript can listen for an event that fires after a customer submits a form.
// Listen for form submissionsdocument.addEventListener('signCustomiserFormSubmitted', function(event) { console.log('Form submitted:', event.detail); // Access form data: event.detail.data // Access customiser ID: event.detail.customiserId // Access domain: event.detail.domain});This event is not sent for stores on the legacy Starter, Standard or Pro plans.
FAQ
How do I receive form submissions?
When a customer submits a form, you receive an email with the customer’s answers.
By default, the email goes to the shop owner’s email address on Shopify, or to the Store owner’s account email on Universal.
If you entered a Receiver Email, the email goes there instead.
To change the text of this email, go to Email Workflows → Custom Design Form Email.
Where can I see the form entries?
Open Quotes from the app admin. This page contains quote requests and form submissions, including AI Sign Designer fallback requests. Select the submission number and form name to open the submission details.
Why is a customer’s email missing?
All new submissions include a required customer email. An older submission can have no email if its form did not collect one.
Open the submission and note the Form and Customiser values.
Select Customisers and open the same customiser.
Go to Features → Forms and select the same form.
Confirm that the form has a required Email field.
The required Email field applies to new submissions only. It cannot add an email to an existing submission or PDF.
